Each network practice is assigned a case manager, and provides its member primary care practices with:

Ongoing education and training;
-
Data analysis on utilization patterns and performance;
-
Continuous quality improvement initiatives; and
-
Centralized disease management program development efforts.
To be eligible for case management, a patient must receive Carolina Access Medicaid, and the patient's primary care physician must be a member of an CCS participating medical practice.
CCS's local case managers then provide patients with:

Home visits;
-
Follow up phone calls;
-
Education in disease process and self management;
-
Connection with community resources; and
-
Education in appropriate use of healthcare resources.
